Once you send your form through, one of our clinicians at Barclay Medical Practice will review your submission and get back to you within 2 working days.

Please note: This service is not suitable for assessing moles. If you have a mole that is changing in size, shape or colour, or you’re worried it may be suspicious, please contact the practice directly to arrange an appropriate review.

Please do not send photos of, or including, intimate areas of the body. Please also note that photos you send us may be seen by staff that are clinical or administrative, and of all genders.

If you’re unable to upload a photo at this time, or if the patient you’re filling out the form for is under the age of 5, please call the practice to schedule a routine appointment instead.
